Baycare Health Partners, Inc.

​A physician-hospital organization (PHO), Baycare® serves the four Baystate Health hospitals and about 175 medical practices with approximately 1,400 physicians in the Pioneer Valley of western Massachusetts. Baycare® has two corporate members, Baystate Health and Greater Springfield IPA, both of which are described below. Baycare® staff provide a health care educational role, quality improvement support and resources, and managed care contracting services to its members, thereby helping physicians manage patient populations through centralized functions that no one physician practice could afford on its own. Baycare® is an alliance of the medical staff and Baystate Health hospitals to collaborate in improving the quality, safety, efficiency, and sustainability of health care in our community. The Baycare® clinical integration programs foster and support collaboration among providers to achieve the IHI Triple Aim. Baycare® staff play an important role in ensuring that best practices are freely shared, practice processes are evaluated and modified as needed, decision-making is evidence-based, and redundancy is minimized.

Pioneer Valley Accountable Care, LLC

​Formed in 2012, Pioneer Valley Accountable Care (PVAC) is a single member limited liability company in Springfield, Massachusetts. It operates as an accountable care organization, participated in the Medicare Shared Savings Program between 2013 and 2015, and has participated in the Next Generation ACO Model since its inception in 2016. PVAC participants care for approximately 40,000 Medicare beneficiaries, which include largely seniors as well as some disabled beneficiaries under age 65 years. About 20 percent of these beneficiaries are dually eligible for Medicare and Medicaid.

Baystate Health Care Alliance, LLC

​In 2017, Baycare® formed a second LLC subsidiary, Baystate Health Care Alliance (BHCA). Representing five community health centers, BHCA has partnered with Health New England, a managed care organization owned by Baystate Health, to form the BeHealthy Partnership, which participates in the MassHealth Accountable Care Organization (ACO) program. These health centers encompass seven sites of care in Springfield and have come together to achieve sustainable delivery system transformation to improve the health and lives of our members and their community.
